Request form DNA diagnostics

Disorders for which we offer molecular diagnostics, are listed on the request form.

Conditions and Shipment

Blood samples (preferred):

  • 2 x 10 ml EDTA blood in plastic tubes (no glass), for neonates at least 3 ml.
  • please put name and date of birth on each tube, we do not accept improperly labelled tubes.
  • ship samples at room temperature. Do not freeze blood samples!
  • samples should arrive in our lab within 5 days after withdrawal.

In case you send in extracted DNA please make sure that the amount of DNA is sufficient (~50 µg). Please put name and date of birth on each tube, we do not accept improperly labelled tubes.

Prenatal testing only after consultation.

Costs
The costs per gene per sample are (fee for 2009):

Mutation not known in the family Postnatal testing, complex analysis ( > 25 amplicons): € 1.400,--
  Postnatal testing, regular analysis (< 25 amplicons) € 725,--
Mutation known in family Postnatal testing, simple analysis (1 amplicon) € 350,--
  Prenatal testing € 800,--

Important: the invoice will be sent to the referring physician. In case another billing address should be used, please indicate this clearly on the request form.
